If you're interested in a virtual preliminary evaluation, it's as convenient as completing the following form and e-mailing us some personal photos.

3. To further expedite your consultation, optionally visit our Forms page, review the details, and send us the completed Questionnaire and Acknowledgement of Receipt of Forms to indicate you have reviewed and understood patient rights, our privacy policies, and practice disclosure.

For non-virtual consultations and procedures we offer two convenient locations.
Please note: surgical procedures are performed in our Marshfield facility.
